The shoe rack I have from Bed Bath Beyond is near to ,[link widoczny dla zalogowanych], which works well for me (and doesn't really fall apart, even after drunkenly stumbling involved with it and taking advantage of it as a steadying device).
Bed Bath Beyond has that is only 3 shelves, but fits most of your requirements (the edges are wood, but the bars where the shoes go are metal). is a bit cheaper,[link widoczny dla zalogowanych], but you'd need to buy a couple of them which means you could have the 4 shelves you want. It is also all metal, unfortunately.
Wow, those are a handful of hard dimensions to locate, particularly the 8" deep. But the Proman Cedar Expandable 2-tier stacking shoe rack meets them all, once you stack enough of them. You can see a picture of them stacked here. And here you go at another site.

I got something similar to that at Fred Meyer,[link widoczny dla zalogowanych], except for the amount of shelves. But,[link widoczny dla zalogowanych], there have been extra holes for more shelves,[link widoczny dla zalogowanych], and also you could probably buy more to include as much as you needed. It was in the past,[link widoczny dla zalogowanych], and that i don't live there anymore,[link widoczny dla zalogowanych], and so i can't verify,[link widoczny dla zalogowanych], however it was the main one on 85th and Greenwood.
